- Microscopic Testicular Sperm Extraction (microTESE) is a medical procedure designed to retrieve sperm directly from the testicular tissue within a man's reproductive system.
- Fertility Enhancement: The procedure is typically recommended when a man faces challenges in producing or releasing sufficient healthy sperm naturally. It's performed with the aim of addressing fertility issues, allowing the man to potentially father a child.
- Testicular Tissue Location: Testicular tissue, where sperm is produced, is located within the two testes. These testes are situated within the scrotum, a small sac positioned behind and under the penis.

- Microsurgery Importance: Vasectomy reversal is more intricate than the original vasectomy and requires microsurgery. Surgeons use a powerful microscope to magnify the vas deferens, the tube that carries sperm, up to 40 times its size. This precision demands specialized skills and knowledge.
- Two Reattachment Methods: Surgeons perform vasectomy reversals in two primary ways:
- Vasovasostomy: This involves sewing together the severed ends of the vas deferens tubes, allowing sperm to flow again.
- Vasoepididymostomy: Here, the vas deferens is attached directly to the epididymis, a small organ at the back of the testicle that holds sperm. This is a more complex procedure and is used if vasovasostomy isn't feasible or likely to work.
- Procedure Choice: The decision between vasovasostomy and vasoepididymostomy depends on whether sperm are observed in the fluid from the vas deferens during surgery. The surgeon often decides during the operation which technique is most suitable. Sometimes a combination of both methods is necessary.
